基于静息态功能磁共振对缺血性卒中伴脑微出血患者认知功能变化的研究
A resting-state functional magnetic resonance-based study of variations in cognitive function in patients with ischemic stroke with cerebral microbleeds

Abstract
Objective:To investigate the relevance between local cerebral functional variability and cognitive function in pa-tients with acute mild ischemic stroke with cerebral microbleeds on the basis of resting state functional magnetic resonance.Meth-ods:A series of 41 patients with acute ischemic stroke with NHISS score≤5 who were admitted to the Department of Neurology of the First Affiliated Hospital of Hainan Medical College during the period from June 2021 October 2021 were recruited into this investigation,all of whom had completed cranial SWI,and were categorized into 16 cases in the Non-CMBs group as well as 25 cases in the CMBs group,based on whether or not the SWI was combined with cerebral microbleeds(CMBs).All patients have completed resting-state functional magnetic resonance scanning and MoCA scale assessment,and by using the regional homogenei-ty(ReHo)as well as amplitude of low frequency fluctuations(ALFF)analysis measures of rs-fMRI.We compared the brain re-gions in which ReHo as well as ALFF differed in the two groups of patients,and performed the ReHo as well as ALFF values of the brain regions with remarkable differences in the MoCA total score as well as its cognitive domain scores for the correlation anal-ysis.Results:In comparison with the Non-CMBs group,the CMBs group had elevated ReHo values in the right precentral gyrus(P<0.05,GRF correction)and diminished ALFF values in the left superior parietal gyrus as well as left precentral gyrus(P<0.05,GRF correction).In the CMBs group,the left precentral gyrus ALFF values were directly connected with orienting power(r=0.445,P=0.026),as well as the ALFF value of the left superior parietal gyrus was directly connected with the MoCA total score(r=0.417,P=0.038),abstraction(r=0.414,P=0.040)as well as orientation(r=0.440,P=0.028)were positively cor-related.After controlling for gender,age,as well as education level,the ALFF value of the left superior parietal gyrus in the CMBs group were directly connected only with orientation(r=0.477,P=0.025).Conclusion:Cerebral microbleeds raise the threat of cognitive impairment in patients with mild ischemic stroke;altered orientation in patients with ischemic stroke with cere-bral microbleeds may be associated with changes in cerebral function in the superior parietal gyrus.关键词
