《伤寒杂病论》实脾理论在原发性肝癌治疗中的应用
Application of the Spleen-Strengthening Theory from Treatise on Cold Damage and Miscellaneous Diseases in Treating Primary Liver Cancer

Abstract
Primary liver cancer(hereinafter referred to as liver cancer)exhibits complex etiology and pathogenesis with diverse symp-toms,yet spleen deficiency persists throughout its progression.The liver and spleen are interconnected and mutually restrictive.Exces-sive liver wood over-restricting the spleen soil leads to spleen weakness,resulting in liver-spleen disharmony.Liver disease may transmit to and burden the spleen,while spleen pathology can also affect the liver.Based on the spleen-strengthening theory from Treatise on Cold Damage and Miscellaneous Diseases"When treating liver disease,recognize that it may transmit to the spleen;thus,prioritize strengthe-ning the spleen"because treating liver cancer necessitates fortifying the spleen.Spleen-strengthening means not only tonifying the spleen,but also regulating the spleen.Direct spleen-strengthening methods include supplementing spleen qi,regulating qi to fortify the spleen,warming the middle energizer to fortify the spleen,regulating the spleen to resolve stagnation,strengthening the spleen to harmo-nize the stomach,warming the middle energizer to replenish deficiency,resolving dampness to invigorate the spleen,nourishing spleen yin and blood,etc.Indirect spleen-strengthening methods include soothing liver qi,clearing liver fire,resolving stasis and unblocking collaterals,clearing liver-gallbladder dampness-heat,nourishing blood to support the liver,enriching yin to soften the liver,etc.When wood(the liver)is regulated,earth(the spleen)becomes robust,and spleen qi regenerates spontaneously.Thus,strengthening the spleen dissolves masses and promotes liver disease recovery.In clinical practice,a stepwise approach is essential,emphasizing the liver-spleen relationship.Especially for advanced liver cancer,attention should be paid to the organic combination of"purging"and"tonify-ing".In a case of deficiency,reinforce it;in a case of excess,purge it.It is necessary to constantly pay attention to protecting the spleen and stomach,and achieve the goal of survival of liver-cancer patients with tumors.关键词
