On"Six-Dimensional"Differentiation and Treatment of Heart-Related Diseases by Ruan Shiwei

Abstract
Professor Ruan Shiwei,based on the"six looks"(heaven,earth,time,person,disease,and syndrome)diagnostic and thera-peutic model he established,has refined the diagnosis and treatment of heart-related diseases from six dimensions:"moving qi;healthy qi and pathogenic factors;root and function;symptom patterns and mechanisms;constitution;seasonal climates".He emphasizes that clini-cal practice must account for the influence of these qi-moving factors)to"adhere to disease mechanisms without neglecting changes of qi."Heart-related diseases often present with mixed deficiency and excess patterns.Treatment should follow the principles of strengthe-ning healthy qi and removing pathogenic factors,distinguishing deficiency and excess and prioritizing the urgency and importance of treating the symptoms and root causes.In emergency situations,treat the symptoms first;for chronic conditions,strengthen the root cause.Depending on the situation,both symptoms and root causes can be treated simultaneously.For deficiency syndrome,methods like tonifying qi,nourishing blood,enriching yin,or warming yang are employed.For excess syndrome,clearing heat,resolving phlegm,pro-moting diuresis,or dispelling stasis are applied.A balanced approach combining root and function,such as regulating blood circulation,calming the spirit,and replenishing qi-blood,ensures normal cardiac function.Heart-related diseases manifest as two categories:Thoracic symptoms can be treated by promoting blood circulation and resolving stasis followed by examining pathogenic mechanism,and tailored with formulas like Immature Bitter Orange,Chinese Chive and Cinnamon Twig Decoction,Red-Guiding Powder,Heart-Supplementing Decoction,etc.Besides,sweating syndrome is a common heart-related complication,so Licorice and Wheat and Jujube Decoction com-bined with Pulse-Engendering Powder is often used to tonify qi,nourish yin,arrest sweating,and generate fluids.Mental symptoms can be addressed using Frigid Extremities Decoction,Gardenia and Soybean Decoction,Licorice and Wheat and Jujube Decoction,Sour Ju-jube Decoction,etc.to stabilize the spirit.Professor Ruan classifies patient constitutions into six types:Normal,cold-delayed,dry-Red,turbid-stagnant,damp-sticky,and fatigue-pale.He advocates aligning treatments with disease rhythms and seasonal variations for optimal efficacy.关键词
