Application of optical navigation system-assisted CT-guided pulmonary nodule puncture positioning and analysis of factors influencing pneumothorax after puncturing

Abstract
Objective:To evaluate the clinical utility of optical navigation system-assisted CT-guided pulmonary nodule puncture positioning before video-assisted thoracoscopic surgery(VATS)and analyze factors influencing post-puncture pneumothorax.Methods:A retrospective analysis was conducted on 104 patients who underwent CT-guided Hook-wire lung nodule puncture positioning before VATS at the Third People's Hospital of Chengdu from June 2023 to December 2023.Patients were divided into the navigation group(n=51,receiving optical navigation-assisted CT-guided localization)and the conventional group(n=53,receiving standard CT-guided localization).Clinical data of patients were compared between the two groups,and multivariate Logistic regression was used to identify pneumothorax risk factors.Results:The navigation group showed shorter minimal distance between the positioning wire and the nodule,higher first-attempt success rate,and higher dose-length product(DLP)than the conventional group(P<0.05).No significant difference in pneumothorax or pulmonary hemorrhage rates was observed between the two groups(P>0.05).Multivariate analysis revealed that increased needle depth into the lung,greater chest wall thickness traversed by the needle,and first-attempt success were protective factors against pneumothorax,while needle proximity to ribs was a risk factor.Conclusion:Optical navigation-assisted CT-guided pulmonary nodule puncture positioning is safe and effective,which has higher puncture precision than conventional puncture.Clinically,optimizing path planning using the navigation system's accuracy,avoiding rib proximity,and ensuring first-attempt success can minimize pneumothorax risks and enhance procedural safety.关键词
