Effect of intelligent pneumoperitoneum pressure management on inflammatory response and postoperative recovery in patients undergoing robot-assisted radical prostatectomy

Abstract
Objective:To explore the effect of intelligent pneumoperitoneum pressure management on inflammatory response and postoperative recovery in patients undergoing robot-assisted radical prostatectomy(RARP),and to provide theoretical basis for clinical practice.Methods:105 patients who received RARP in Tangdu Hospital,Fourth Military Medical University from December 2021 to February 2024 were selected.They were divided into the intelligent group(n=53,receiving intelligent pneumoperitoneum pressure management)and the traditional group(n=52,receiving traditional pneumoperitoneum pressure management).Pneumoperitoneum-related metrics,vital signs,hemodynamics,inflammatory markers,postoperative recovery indicators,and complication rates were compared between the two groups.Results:Compared to the traditional group,the intelligent group had shorter times to pneumoperitoneum establishment and maintenance,along with superior pressure stability(P<0.05).5 min after anesthesia,no significant differences existed between the two groups in systolic/diastolic blood pressure(SBP/DBP)or respiratory rate(RR)(P>0.05).However,SBP and RR were lower in the intelligent group at 5 min after insufflation/desufflation(P<0.05),while no differences were observed in DBP between the two groups at each timepoint(P>0.05).Central venous pressure(CVP)and cardiac output(CO)of the two groups were comparable at 5 min after anesthesia(P>0.05),but CVP was lower in the intelligent group at 5 min after insufflation/desufflation(P<0.05).Compared with that before surgery,WBC,CRP,and IL-6 levels were increased in both groups at 48 h after surgery,and they were significantly lower in the intelligent group than those in the traditional group(P<0.05).The intelligent group showed earlier times to first flatus,defecation,ambulation,catheter removal,and hospital discharge than the traditional group(P<0.05).No significant difference was observed in total complication rates(P>0.05).Conclusion:Application of intelligent pneumoperitoneum pressure management in RARP can significantly shorten pneumoperitoneum duration,enhance pressure stability,mitigate perturbations in vital signs and hemodynamics,attenuate inflammatory responses,and accelerate postoperative recovery,which is worthy of clinical implementation and promotion.关键词
