基于慢性病适应护理中域理论的桂枝芍药知母汤中医经方护理治疗类风湿关节炎病人的临床效果
Clinical efficacy of TCM classical formula nursing care with Guizhi Shaoyao Zhimu Decoction based on the middle-range theory of chronic illness adaptation in patients with rheumatoid arthritis

Abstract
Objective:To explore the clinical effects of TCM classical formula nursing care with Guizhi Shaoyao Zhimu Decoction based on the middle-range theory of chronic illness adaptation in patients with rheumatoid arthritis(RA).Methods:Guided by the middle-range theory of chronic illness adaptation,100 RA patients diagnosed in the rheumatology and immunology department of Jiangsu Provincial Hospital of Chinese Medicine from January to October 2024 were selected using convenience sampling.Follow-up was conducted 4 weeks after discharge.Patients were randomly divided into an observation group(n=50)and a control group(n=50).The control group received oral methotrexate plus routine nursing care.The observation group received TCM classical formula nursing care(Guizhi Shaoyao Zhimu Decoction)plus oral methotrexate and routine nursing care.Improvements in tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),anti-cyclic citrullinated peptide(CCP)antibody,interleukin-6(IL-6),immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM),rheumatoid factor(RF),and erythrocyte sedimentation rate(ESR)levels were compared between the two groups,along with morning stiffness duration,number of swollen joints,Visual Analogue Scale(VAS)score,and nursing satisfaction score.Results:After treatment,CRP,IL-6,TNF-α,ESR,and IgA levels in the observation group were significantly lower than those in the control group(P<0.05).The observation group also had shorter morning stiffness duration,milder pain intensity,and higher nursing satisfaction scores compared to the control group(P<0.05).Conclusions:The nursing intervention using the TCM classical formula Guizhi Shaoyao Zhimu Decoction can effectively improve inflammatory status,alleviate pain,and enhance nursing satisfaction in RA patients.It helps healthcare workers screen high-risk populations for maladaptation to chronic illness and promotes better chronic illness adaptation behaviors.关键词
