2型糖尿病病人行内镜下黏膜剥离术发生低血糖的影响因素分析
Analysis of influencing factors for hypoglycemia in type 2 diabetes patients undergoing endoscopic submucosal dissection

Abstract
Objective:To analyze the influencing factors for hypoglycemia in type 2 diabetes mellitus(T2DM)patients undergoing endoscopic submucosal dissection(ESD).Methods:A retrospective analysis was performed on the electronic medical records of 140 T2DM patients who underwent ESD in the department of gastroenterology at the Affiliated Gaochun Hospital of Jiangsu University between January 2021 to December 2023.Among them,24 patients experienced hypoglycemia,while 116 did not.Logistic regression analysis was used to identify the influencing factors for hypoglycemia in diabetic patients undergoing ESD.Results:The incidence of hypoglycemia in T2DM patients undergoing ESD was 17.1%.Logistic regression analysis demonstrated that diabetes duration≥10 years,standard deviation of blood glucose(SDBG)≥3 mmol/L,body mass index(BMI)<18.5 kg/m²,perioperative fasting time≥36 hours,and inappropriate fluid replacement during the perioperative fasting period were influencing factors for hypoglycemia in diabetic patients undergoing ESD.Conclusion:The incidence of hypoglycemia is relatively high in T2DM patients undergoing ESD.Clinical nursing staff should pay close attention to the patient's diabetes duration,SDBG,BMI,perioperative fasting time,and fluid replacement status during the perioperative fasting period.preoperative assessment,early warning,and effective interventions should be implemented to reduce the incidence of hypoglycemia in T2DM patients undergoing endoscopic surgery.关键词
