Changes in Serum SAA,CCL-18 and D-D in Patients with Stable Phase of Chronic Obstructive Pulmonary Disease and Their Relationship with Pulmonary Ventilation Status and Prognosis

Abstract
Objective To analyze the changes in serum amyloid A(SAA),chemokine ligand 18(CCL-18)and D-dimer(D-D)in patients with stable phase of chronic obstructive pulmonary disease(COPD)and their relationship with pul-monary ventilation status and prognosis.Methods In total,156 COPD patients admitted from January 2020 to December 2023 were enrolled as the observation group,and 50 healthy people undergoing physical examination during the same period were selected as the control group.The serum CCL-18,SAA,and D-D levels of the two groups were detected,the pulmonary ventilation function of the observation group was evaluated,and the prognosis results within 6 months were calculated.The levels of serum SAA,CCL-18 and D-D were compared in patients experiencing stable phase of COPD with different ventila-tion functions and between the observation group and the control group,and the relationship between the levels of serum SAA,CCL-18 and D-D in patients with stable phase of COPD and pulmonary ventilation function and prognosis was analyzed.Re-ceiver operating characteristic(ROC)curve was used to analyze the predictive value of serum SAA,CCL-18 and D-D levels for the prognosis of patients with stable phase of COPD.Results Six patients in the observation group were excluded because of loss to follow-up,and 150 patients were finally included in the study,including 20 patients with mild airflow limitation,50 pa-tients with moderate airflow limitation,57 patients with severe airflow limitation,and 23 patients with extremely severe airflow limitation.Thirty-three patients had poor prognosis.The serum CCL-18,SAA,and D-D levels in the observation group were higher than those in the control group(P<0.01).Serum CCL-18,SAA,and D-D levels were the lowest in mild group,followed by moderate group,severe group and extremely severe group(P<0.05).The levels of serum CCL-18,SAA,and D-D in the poor prognosis group were higher than those in the good prognosis group(P<0.01).Spearman correlation analysis showed that serum SAA,CCL-18 and D-D levels in patients with stable phase of COPD were positively correlated with pulmonary ventilation function and poor prognosis(P<0.01).ROC curve analysis showed that serum SAA,CCL-18 and D-D levels in patients with stable phase of COPD had predictive value for prognosis(P<0.01).Conclusions The levels of SAA,CCL-18 and D-D in patients with stable phase of COPD are higher than those in healthy people,and the degree of increase is significantly correlated with lung ventilation status and prognosis.关键词
