Imaging Characteristics and Literature Review of Abnormal Systemic Artery Supplying Normal Lower Lung Lobe

Abstract
Objective To investigate the clinical manifestations and imaging characteristics of abnormal systemic artery supplying normal lower lung lobe(ASANLL).Methods A retrospective analysis was performed on the imaging examinations and diagnosis and treatment process of 9 patients diagnosed with ASANLL in our hospital.PubMed,Wanfang database,and CHKD Journal Full text Database up to December 2024 were searched for relevant literature on ASANLL to analyze and learn the imaging manifestations of the disease.Results Among the 9 patients in this group,there were 8 males and 1 female.Four patients presented with unexplained hemoptysis as the main symptom,two patients were detected during examinations for cough and expectoration,and three patients were incidentally found during physical examination without obvious clinical symptoms.Enhanced chest CT scan or bronchial artery CT angiography(CTA)showed that abnormal systemic circulation vessels originated from the descending aorta in 8 patients and the celiac trunk in 1 patient,with tortuous and dilated lumens.The blood supply was to the basal segment of the left lower lung lobe in 7 patients and the basal segment of the right lower lung lobe in 2 patients.Six patients were misdiagnosed as pulmonary sequestration,among which 2 patients underwent surgical treatment and 4 patients received symptomatic treatment;the remaining 3 patients were diagnosed as ASANLL,all of which were given symptomatic treatment.After follow-up observation,the 2 patients treated by surgery recovered well,and the other 7 patients were in good general condition.A total of 22 Chinese literature and 51 foreign literature were retrieved,reporting 135 patients in total.Conclusion ASANLL is relatively rare,with hemoptysis as the main clinical manifestation.Enhanced chest CT and bronchial artery CTA can be used for multi-planar reconstruction to confirm the diagnosis.关键词
