Abstract
Objective To investigate the application value of endoscopic trabeculectomy combined with endo-scopic cyclophotocoagulation(ECP)and phacoemulsification and intraocular lens implantation(PEI)for the treatment of coexisting advanced primary open angle glaucoma(POAG)and cataract.Methods A retrospective case-control study.was conducted on 65 patients with advanced POAG and cataract,who were treated at Xuzhou Municipal Hospital Affiliated to Xuzhou Medical University from July 2021 to December 2022.They were assigned into two groups:A group and B group.Endoscopic trabeculectomy combined with ECP and PEI was performed in 33 cases in the A group,while ECP combined with PEI was performed in 32 cases in the B group.The preoperative and postoperative intraocular pressure(IOP),types of IOP-lowering medications used,success rate,and postoperative complications were compared between the two groups in the present study.Results After the operation,intraocular hypertension in both groups returned to normal level.There was no significant difference(P>0.05)between the two groups in IOP during the at:first month after the operation,while IOP was significantly reduced(all P<0.01)from at the third month to the twelfth month after the operation in the A group as compared with the B group.The types of IOP drugs required in the A group after the operation were significantly lower than those in the B group(P<0.01).There was no difference in the cumulative success rate between the two groups(P>0.05),and the complete success rate of the A group was significantly higher than that of the B group(P<0.01).The inci-dence of early postoperative hyphema in the A group was higher than that in the B group(P<0.01),while this postopera-tive complication could be recovered within 2 weeks after the surgery.There was no significant difference(P>0.05)in the incidence of transient intraocular hypertension and ciliary body detachment between the two groups.Conclusions The new combination surgery of endoscopic trabeculectomy,ECP,and PEI showed a superior efficacy in reducing IOP com-pared to ECP plus PEI for the treatment of advanced POAG patients with cataract.This new surgery above is more benefi-cial to reach the target IOP of patients with advanced POAG,and exhibits good application value in the treatment of pa-tients with coexisting advanced POAG and cataract.关键词
