VEGF/PEDF、ABCA1在新生血管性青光眼患者中的表达及临床意义
Expression and clinical significance of VEGF/PEDF and ABCA1 in patients with neovascular glaucoma

Abstract
Objective To investigate the expression and clinical significance of the vascular endothelial growth factor(VEGF)/pigment epithelial derived factor(PEDF)value and adenosine triphosphate(ATP)binding cassette trans-porter A1(ABCA1)in patients with neovascular glaucoma(NVG).Methods The prospective case-control study.A total of 108 NVG patients(108 eyes)who underwent treatment in our hospital from May 2022 to May 2024 were regarded as the test group.According to the Teich grading criteria,they were grouped into 59 patients in grades Ⅰ～Ⅱ,49 patients in grades Ⅲ～Ⅳ.Additionally,108 cataract patients(108 eyes)during the same period were selected as the reference group.Enzyme linked immunosorbent assay(ELISA)was applied to detect levels of VEGF,PEDF,and ABCA1,and the VEGF/PEDF value was calculated.The Spearman and Pearson correlation were applied to analyze the correlation between VEGF,PEDF,ABCA1,and VEGF/PEDF value with Teich grading and intraocular pressure(IOP).The influencing factors of NVG were analyzed using multiple factor logistic regression.The receiver operating characteristic(ROC)curve was con-structed to explore the diagnostic value of serum VEGF/PEDF value and ABCA1 for NVG.Results The serum levels of VEGF,ABCA1,and VEGF/PEDF value in the test group were greatly higher than those in the reference group(all P<0.05),while the serum PEDF level in the test group was greatly lower than that in the reference group(P<0.05).The se-rum levels of VEGF,ABCA1,and VEGF/PEDF value in patients with grade Ⅲ～Ⅳ NVG were greatly higher than those in patients with grade Ⅰ～Ⅱ(all P<0.05),while the serum PEDF level in patients with grade Ⅲ～Ⅳ NVG was greatly lower than that in patients with grade Ⅰ～Ⅱ(P<0.05).According to Spearman correlation analysis,serum VEGF,ABCA1 levels,and VEGF/PEDF value were positively correlated with Teich grading(all P<0.05),while serum PEDF level was negatively correlated with Teich grading(P<0.05).According to Pearson correlation analysis,serum VEGF,ABCA1 levels,and VEGF/PEDF value were positively correlated with IOP(all P<0.05),while serum PEDF level was negatively correlated with IOP(P<0.05).Multivariate logistic regression showed that VEGF,ABCA1,VEGF/PEDF value,IOP and diabetes were risk factors for NVG(all P<0.05),and PEDF was a protective factor for NVG(P<0.05).According to the ROC curve,the area under the curve(AUC)value for diagnosing NVG with VEGF/PEDF value was 0.788,the AUC value for diagnosing NVG with serum ABCA1 level was 0.717,and the AUC value for diagnosing NVG with the combination of the two was 0.834.The combination of the two was better than their respective individual diagnoses(Zcombination vs.VEGF/PEDF=2.801,Zcombination vs.ABCA1=2.697,all P<0.05).Conclusions The VEGF/PEDF value and ABCA1 level are higher in the serum of NVG patients and are influencing factors for the occurrence of NVG.The combined detection of the two has high diagnostic value for NVG.关键词
