Abstract
Objective To analyze the influencing factors of short-term intraocular pressure(IOP)elevation in eyes after intravitreal injection of anti-vascular endothelial growth factor(VEGF)drugs.Methods A retrospective obser-vational study was conducted.A total of 74 patients(92 eyes,178 injection treatments)with retinal diseases treated with intravitreal injections of anti-VEGF drugs in our hospital from January to November 2023 were included,IOP was mea-sured before injection and at 10 minutes,30 minutes,2 hours,and 1 day after injection.Based on the IOP elevation at 10 minutes after injection,the patients were divided into two groups:the IOP elevation group(IOP increased≥10 mmHg compared to pre-injection)and the IOP stable group(IOP increased<10 mmHg compared to pre-injection).T-test was used to compare age,number of injections,preoperative IOP,and IOP at various times after injection between the two groups.The chi-square test was used to compare the differences in gender,ocular laterality,type of anti-VEGF drug,types of disease,lens status,and vitreous reflux after injection between the two groups.Logistic regression was used to ana-lyze the factors affecting the elevation in IOP at 10min after injection.A multivariate analysis was conducted considering the impact of multiple injections in the same eye by the same person,with the eye numbers included as a variable in the equation.Results The mean IOP before the surgery,and at 10 min,30 min,2 h,and 1 d after injection were(17.15±3.45)mmHg,(23.40±7.42)mmHg,(20.20±4.38)mmHg,(19.41±2.76)mmHg,(16.90±3.53)mmHg,respectively.The meanIOP increased by(6.24±5.84)mmHg,(3.05±4.05)mmHg,(2.26±3.15)mmHg,and(-0.24±1.51)mmHg,respec-tively.In 178 injection treatments,IOP elevation occurred in 40(22.47%)and was stabled in 138(77.53%).Univariate analysis showed that there were no statistically significant differences in age,gender,ocular laterality,type of anti-VEGF drug,type of diseases,lens status,and number of injections between the two groups(P>0.05).There were statistically significant differences in preoperative IOP,IOP at various times after injection,and vitreous reflux after injection(P<0.05).Multivariate analysis showed that IOP at 10 min after injection was positively correlated with preoperative IOP and negatively correlated with vitreous reflux after injection.The higher the preoperative IOP and the absence of vitreous reflux after injection,the greater the chance of an increase in IOP after injection,and the difference was statistically significant(P<0.05).Conclusions The main factors affecting the short-term elevation of IOP after intravitreal injection of anti-VEGF drugs are preoperative IOP and vitreous reflux after injection.The higher the preoperative IOP and the absence of vitreous reflux after injection,the greater the risk of increased IOP after injection.关键词
