不同剂量艾司氯胺酮联合丙泊酚和舒芬太尼对小儿斜视手术全麻诱导期眼内压的影响
Effect of different doses of esketamine combined with propofol and sufentanil on intraocular pressure during general anesthesia induction in children undergoing strabismus surgery

Abstract
Objective To evaluate the effects of different doses of esketamine combined with propofol and sufent-anil on intraocular pressure during general anesthesia induction in children undergoing strabismus surgery.Methods A prospective case-control study.A total of 120 children who received unilateral strabismus surgery under general anesthesia in Baoding First Central Hospital from January 2022 to March 2023 were selected,and randomly enrolled in the esket-amine 0.25 mg/kg group(E0.25 group),esketamine 0.5 mg/kg group(E0.5 group),or placebo group(P group),with 40 children in each group.The E0.25 group was given intravenous esketamine 0.25 mg/kg.The E0.5 group was given esketamine 0.5 mg/kg intravenously.The Placebo group was given 0.9%sodium chloride injection 2 ml intravenously.The systolic blood pressure(SBP),diastolic BP(DBP),heart rate(HR)and intraocular pressure were recorded before anesthesia in-duction(T0),1 min after anesthesia induction(T1),immediately after laryngeal mask insertion(T2),and 2 min after la-ryngeal mask insertion(T3).The intraocular pressure was measured by the ophthalmologist using a handheld applanation tonometer.The postoperative nausea and vomiting,delayed recovery,agitation during recovery and length of postanesthe-sia care unit(PACU)stay were recorded.Results At T1 time point,the intraocular pressure in the E0.5 group was signifi-cantly elevated compared to the P group(P<0.05).The intraocular pressure at T1-T3 was significantly lower in all three groups compared to their respective values at T0(P<0.05).At the T1 time point,the heart rate in the E0.5 group was nota-bly higher than in the P group.Additionally,the DBP at T1 in both the E0.25 and P groups was significantly reduced com-pared to their T0 values(P<0.05).There were no significant differences in the rates of complication,PACU stay duration across the three groups(P>0.05).Conclusions 0.5 mg/kg esketamine combined with propofol and sufentanil will not el-evate intraocular pressure compared to baseline in pediatric strabismus surgery and did not increase the incidence of post-operative adverse reactions.关键词
