布比卡因脂质体行腹股沟韧带上髂筋膜阻滞对全髋关节置换术后镇痛效果的影响
Effect of supra-inguinal fascia iliaca block with liposomal bupivacaine on analgesia after total hip arthroplasty

Abstract
Objective To compare the analgesic effect of liposomal bupivacaine(LB)and ropivacaine for supra-inguinal fascia iliaca block(SIFIB)in patients undergoing total hip arthroplasty(THA).Methods Sixty patients of either gender,aged 62-81 years old,with a body weight of 48-82 kg,of American Society of Anesthesiologists(ASA)Physical Status classification Ⅰ or Ⅱ,who underwent unilateral THA at Zibo 148 Hospital from June to December 2023 were selected as the study objects.The patients were divided into liposo-mal bupivacaine group(group B,n=30)and ropivacaine group(group R,n=30)by random number table method.SIFIB was per-formed in both groups.The postoperative motor block,visual analogue scale(VAS)at rest and during activity at 12,24,36,48,60 and 72 h after operation,the pressing times of patient controlled intravenous analgesia(PCIA),the number of cases of tramadol rescue anal-gesia,the first time to get out of bed after operation,satisfaction score of postoperative analgesia and incidence of postoperative nausea and vomiting,dizziness,pruritus and respiratory depression were recorded.Results There was no motor block in both groups after operation.The resting VAS scores of group B at 24 h and 36 h after operation were lower than those of group R(P＜0.05).During the activity period,VAS scores of group B were lower than those of group R at 24,36,48,60 and 72 h(P＜0.05).The pressing times of PCIA,the num-ber of tramadol rescue analgesia,and the first ambulation time in group B were lower than those in group R(P＜0.05),and the postop-erative analgesia satisfaction score was higher than that in group R(P＜0.05).The incidence of nausea and vomiting in group B was low-er than that in group R(P＜0.05).Conclusion In THA,SIFIB with liposomal bupivacaine can provide analgesia for patients for up to 72 h and reduce postoperative adverse reactions.关键词
