Effect of Remote Ischemic Preconditioning on Postoperative Delirium in Patients Undergoing Elective Non-Cardiac Surgery:A Meta-Analysis

Abstract
Objective To evaluate the effects of remote ischemic preconditioning(RIPC)on postoperative delirium(POD)in patients undergoing elective non-cardiac surgery through a systematic review and Meta-analysis.Methods Databases including CNKI,Wanfang,VIP,the China Biomedical Literature Service System,PubMed,Embase,Web of Science,and the Co-chrane Library were systematically searched from their inception dates to September 13,2024.Randomized controlled trials(RCTs)were included if they enrolled patients undergoing elective non-cardiac surgery,applied RIPC prior to surgery in the inter-vention group,and had no RIPC in the control group.Data were analyzed using RevMan 5.3.Results A total of 10 RCTs invol-ving 656 patients were included,with 328 in the RIPC group and 328 in the control group.There were no significant differences be-tween the two groups in age,sex,duration of surgery,body mass index,or American Society of Anesthesiologists classification.Com-pared with the control group,the RIPC group showed a decreased incidence of POD within 7 days(RR=0.45,95％CI 0.28～0.72,P＜0.001)and a shorter duration of POD(MD=-0.80 days,95％CI-1.57～-0.03,P=0.04).In addition,the serum S100β protein level at 24 hours post-surgery was significantly lower in the RIPC group than in the control group(MD=-0.29 pg/L,95％CI-0.44～-0.13,P＜0.001),as was the serum neuron-specific enolase level(MD=-2.99 μg/L,95％CI-5.56～-0.42,P=0.02).No adverse events were reported in either group.Conclusion Preoperative RIPC can reduce the in-cidence of POD within 7 days in patients undergoing elective non-cardiac surgery,with no obvious adverse events observed.关键词
