血清GLP-1、GAS6水平对妊娠期糖尿病孕妇自发性早产的预测价值
Predictive Value of Serum GLP-1 and GAS6 Levels for Spontaneous Preterm Delivery in Pregnant Women with Ges-tational Diabetes Mellitus

Abstract
Objective To investigate the predictive value of serum levels of glucagon-like peptide(GLP-1)and growth arrest specific protein 6(GAS6)for spontaneous preterm delivery in pregnant women with gestational diabetes mellitus(GDM).Methods A total of 56 GDM pregnant women with spontaneous preterm delivery admitted to the hospital from October 2021 to October 2023 were selected as the preterm delivery group,112 pregnant women with full-term GDM were in the full-term group,56 healthy pregnant women were selected as the control group.Serum GLP-1 and GAS6 levels of preterm,full-term and control groups were compared,and clinical data of preterm and full-term groups were compared.The relationship between serum GLP-1,GAS6,GDM and spontaneous preterm birth was analyzed by Logistic.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of the two serum factors on spontaneous preterm delivery.Results Compared with the control group,both pre-term and full-term groups had lower serum GLP-1 levels and higher GAS6 levels(P＜0.05).Compared with the full-term group,the serum GLP-1 level in the preterm group was lower and the GAS6 level was higher(P＜0.05).The previous number of prema-ture births was ≥2 times,family history of diabetes,history of abortion,OGTT 0 h blood glucose level,HbA1c,white blood cell count and serum GLP-1 and GAS6 levels in the preterm group were higher than those in the full-term group(P＜0.05).Multivari-ate Logistic regression model showed that history of abortion,the previous number of premature births was≥2 times,OGTT 0 h≥5.1 mmol/L,white blood cell count＞10.0 × 109/L,high GAS6 and low GLP-1 were independent risk factors for spontaneous pre-term delivery in pregnant women with GDM(P＜0.05).ROC curve showed that the AUC of serum GLP-1 and GAS6 alone in pre-dicting spontaneous preterm delivery in GDM pregnant women was 0.782 and 0.641,respectively,and the AUC of combined pre-diction was 0.813.The combined prediction efficiency was better than that of single prediction(P＜0.05).Conclusion Serum GLP-1 and GAS6 in pregnant women with GDM are closely related to spontaneous preterm birth.Serum GLP-1 and GAS6 have cer-tain predictive value for the risk of spontaneous preterm birth in pregnant women with GDM,and the combined prediction value is higher.关键词
