摘要

Abstract

Objective:To compare the dosimetric advantages between coplanar and non-coplanar volumetric modulated arc therapy(VMAT)in treatment of thymic tumors.Methods:Fourteen patients with thymus tumor were randomly enrolled to develop coplanar and non-coplanar VMAT plans,respectively.PTV and dosimetric parameters of organs at risk were com-pared between the two groups,including:target conformity index(CI);dose homogeneity index(HI);V30,V20,V5 of lung exposure ≥30 Gy,20 Gy,5 Gy volume;mean lung dose(MLD)of the whole lung;V40,V30 and the mean exposure dose Dmean of the heart;the minimum dose D1cc within the 1cc volume of maximum exposure of the esophagus and trachea;the maximum exposure dose Dmax of the spinal cord.Paired sample t-test or Wilcoxon rank sum test was used for statistical analy-sis,and P<0.05 was considered as statistically significant difference.Results:CI and HI of coplanar plan PTV were better than those of non-coplanar plan(P<0.05).V30 of both lungs in the coplanar plan were better than that in the non-coplanar plan(P<0.05),while V5 and MLD were higher than those in the non-coplanar plan(P<0.05).The differences in V20,however,were statistically insignificant(P=0.875).The Dmean of the esophagus in the coplanar plan was significantly lower than that in the non-coplanar plan(P<0.05),and the monitor units were significantly higher for the coplanar plan(P<0.05).However,no statistically significant differences were observed between the plans for the V40,V30 and Dmean of the heart,the D1cc of the esophagus,D1cc and Dmean of the trachea and Dmax of the spinal cord(P>0.05).Conclusion:Non-coplanar plans for thymic tumors have certain advantages in protecting the lungs,heart,and trachea,but the dose to the esophagus is higher.
