异常肌肉反应波个性化监测方案在面肌痉挛患者微血管减压术中的应用
Clinical application of a personalized AMR wave monitoring protocol in microvascular decompression for hemifacial spasm patients

Abstract
Objective To observe the clinical effect of a personalized abnormal muscle response(AMR)wave moni-toring protocol in microvascular decompression(MVD)for patients with hemifacial spasm(HFS).Methods This retro-spective study included 120 patients with HFS treated with botulinum toxin(BTX)who subsequently underwent MVD.They were divided into the observation group(n=60,personalized AMR wave monitoring protocol)and control group(n=60,routine AMR wave monitoring protocol).AMR wave amplitude and latency,intraoperative disappearance rates,post-operative efficacy,and incidence of mild facial palsy were compared between the two groups.Results In the observation group,AMR wave amplitude and latency were(0.84±0.06)mV and(4.19±0.19)ms,respectively,and they were(0.63±0.04)mV and(5.11±0.15)ms in the control group,with statistically significant difference between these two groups(all P<0.05).AMR wave disappearance during surgery occurred in 41 cases(68.33%)in the observation group versus 21 cases(35.0%)in the control group(P<0.05).AMR latency prolongation or incomplete disappearance oc-curred in 19 cases(31.67%)and 39 cases(65.00%),respectively,in the observation and control groups(P<0.05).Im-mediate postoperative outcomes in the observation group were recovery in 35 cases(58.33%),significant improvement in 10(16.67%),slight improvement in 4(6.67%),and no improvement in 11(18.33%)cases,while the control group had 20(33.33%),10(16.67%),5(8.33%),and 25(41.67%)cases,respectively.At 3-month follow-up,outcomes in the observation group were complete recovery in 40 cases(66.67%),significant improvement in 10(16.67%),slight improvement in 6(10.00%),and no improvement in 4 cases(6.67%).The corresponding figures in the control group were 30(50.00%),9(15.00%),5(8.33%),and 16(26.67%)cases,respectively;there were significant difference in the each efficacy grade between the two groups at both time points(all P<0.05).Mild facial palsy(House-Brackmann grade Ⅱ)occurred in 3 cases in the observation group(all recovered within 3 months)and 5 cases in the con-trol group(3 recovered within 3 months,2 remained grade Ⅱ),with no significant difference between groups(P>0.05).Conclusion Applying a personalized AMR monitoring protocol in MVD for HFS patients after BTX treatment can im-prove intraoperative monitoring quality and effectively enhance surgical efficacy.关键词
