针刺联合经皮耳迷走神经刺激对缺血性卒中患者上肢运动功能的影响
Effect of acupuncture combined with transcutaneous auricular vagus nerve stimulation on upper limb motor function in patients with ischemic stroke

Abstract
Objective To observe the clinical efficacy of acupuncture combined with transcutaneous auricular vagus nerve stimulation(taVNS)in treating upper limb motor dysfunction following stroke.Method A total of 68 patients with ischemic stroke were randomly divided into an observation group and a control group,with 34 cases in each group.On the basis of standard treatment(internal medicine and rehabilitation training),the control group received acupuncture combined with sham taVNS,while the observation group received acupuncture combined with taVNS.The change of latency and amplitude of motor evoked potentials(MEP)of the abductor pollicis brevis(APB)on the paretic side,Fugl-Meyer assessment for upper extremity(FMA-UE),Wolf motor function test(WMFT),and the modified Barthel Index(MBI)were observed in the two groups before and after treatment.Result After treatment,both groups showed a significant decrease in APB MEP latency and increase in amplitude on the paretic side(P＜0.05),with the observation group showing greater improvements in APB MEP latency and amplitude compared to the control group(P＜0.05).Scores of FMA-UE,WMFT,and MBI were significantly increased in both groups after treatment(P＜0.05),and were significantly higher in the observation group than in the control group(P＜0.05).Conclusion On the basis of standard treatment,acupuncture combined with taVNS can enhance excitability of motor pathways and improve upper limb motor function and activities of daily living in patients with ischemic stroke.关键词
