Abstract
Objective To observe the clinical efficacy of acupuncture combined with Western medicine in the treatment of gastroparesis syndrome after hepatobiliary surgery and its effect on serum gastrin(GAS)and vasoactive intestinal peptide(VIP)levels.Method Eighty patients with gastroparesis syndrome after hepatobiliary surgery were randomly divided into an observation group and a control group,with 40 cases in each group.Both groups were treated with conventional therapy,the control group was treated with domperidone,while the observation group was treated with acupuncture on the basis of the control group.The recovery of gastrointestinal dynamics(time of normalization of bowel sounds,time of first bowel movement,time of gastrointestinal decompression and drainage,and time of resumption of voluntary feeding)were observed in the two groups,and the levels of brain-gut peptides(serum GAS and VIP)and revised gastroparesis cardinal symptom index(GCSI-R)scores were compared between the two groups before and after treatment,and the correlation between post-treatment serum GAS and VIP levels and gastrointestinal motility and GCSI scores were analyzed.The clinical efficacy and incidence of adverse reactions were compared between the two groups.Result The total effective rate of the observation group was 95.0%,which was higher than 77.5%of the control group(P<0.05).The time of normalization of bowel sounds,the time of first defecation and the time of resumption of spontaneous feeding in the observation group were shorter than those in the control group(P<0.05),and the volume of gastrointestinal decompression and drainage was smaller than that in the control group(P<0.05).After 2 weeks of treatment,after 4 weeks of treatment and after treatment,the GCSI-R scores of the two groups were lower than those before treatment(P<0.05),and those of the observation group were lower than those of the control group(P<0.05).Serum GAS level was negatively correlated with the time of normalization of bowel sounds,the time of first defecation,the time of resumption of spontaneous feeding,gastrointestinal decompression and drainage,and GCSI score(P<0.05),while serum VIP level was positively correlated with the time of normalization of bowel sounds,the time of first defecation,the time of resumption of spontaneous feeding,gastrointestinal decompression and drainage,and GCSI score(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Cerebral peptide is closely related to gastrointestinal dynamics and clinical symptoms in patients with gastroparesis syndrome after hepatobiliary surgery.On the basis of conventional treatment,acupuncture combined with Western medicine can improve the index of brain-gut peptides and promote the recovery of gastrointestinal dynamics and function of patients with gastroparesis syndrome after hepatobiliary surgery,without increasing the adverse effects.关键词
