Abstract

Retained surgical item(RSI)accidents occasionally occur in medical malpractice cases.These incidents can lead to wound infections,prolong hospitalization,delayed postoperative recovery,and even severe complications such as tumor formation,ultimately endangering the patient's life.Although safety measures to prevent RSI are widely implemented in the United States and other countries,these accidents persist without an effective solution.RSI can be categorized into soft(gauze,padding,and towels)and hard(instruments,needles,and blades)based on material properties,in which the most common leftover is medical textiles,especially medical gauze,which are soft and easy to fit the wounds,so that they absorb blood to deform during surgery or nursing,increasing the risk of retention. Currently,the use of technology to intervene and improve the counting accuracy of medical gauze is a key research direction for the effective prevention of RSI.The primary methods include manual counting,X-ray inspection,barcode or data matrix code,and radio frequency identification(RFID).The manual counting method is time-consuming,as it requires staff to separate gauze pieces stacked together after surgery,making it prone to human error;X-ray inspection provides clear visualization,but it entails high costs,prolonged processing time,and potential radiation exposure for patients;the use of barcode or data matrix code systems demands precise alignment for scanning each label,which extends counting time and fails to detect retained surgical items inside the patient's body.In contrast,RFID technology uses radio magnetic signals to read tag information on the target.The compact transmitters and tags do not interfere with normal surgical procedures.RFID boasts advantages such as long recognition distance,durability in harsh environments,high detection accuracy,and rapid multi-tag scanning.Additionally,its signal penetration reduces the time of detecting gauze before and after surgery,and reduces the incidence of accidents of left-behind surgical items. RFID system consists of three parts:RFID tag,RFID reader and background database,forming a non-contact automatic identification system.RFID tags are categorized into passive and active types according to the presence or absence of power supply.Compared with active RFID tags,passive RFID tags require no internal power supply,featuring compactness and inexpensiveness,so passive RFID tags are widely used in medical gauze management.In existing studies,multiple studies have adopted passive RFID tags or RFID tag-based surgical detection systems to identify retained medical gauze.These solutions demonstrate high detection accuracy,enabling prompt identification of retained gauze within the body during surgical procedures,thereby enhancing surgical safety.At the same time,there are also studies on the factors affecting the detection accuracy of RFID tags.These results show that the presence of liquids(blood,body fluids,and water)affects the detection rate of RFID technology to detect medical gauze;implantable pacemakers and defibrillators,or temporary epicardial pacemakers,may cause interference or significantly affect detection results.When two RF chips are in proximity or metal objects(such as surgical instruments including tweezer handles,scalpels,and catheter needles)are present near the chips,they can interfere with radio wave transmission between RFID tags and readers,compromising detection outcomes. However,existing research reveals that radiofrequency identification tags designed for intelligent detection of medical gauze predominantly feature circular or oval planar configurations.These relatively bulky tags exhibit limited flexibility,restricting natural bending during gauze manipulation in clinical settings.Furthermore,current integration methods relying on adhesive bonding or suturing pose potential safety concerns due to detachment incidents. RFID technology offers wireless operation,passive functionality,and high detection accuracy,making it a fast,safe,and efficient method for medical gauze detection.Multiple studies demonstrate that this technology achieves 100%detection accuracy,indicating promising application prospects in medical gauze management.RFID technology also demonstrates significant potential in medical gauze intelligent management and delivers notable results.Compared with other representative technologies,RFID technology excels in reducing the number of RSI accidents in healthcare due to its superior detection accuracy and sensitivity.While multiple studies confirm its high identification accuracy,the technology still confronts several challenges:bulky tag dimensions,elevated costs,and integration methods like taping or stitching that compromise gauze flexibility and comfort.Currently,the application of RFID technology in the intelligent management of medical gauze is gradually gaining attention,and future development will prioritize three key areas:enhancing detection capabilities and interference resistance,achieving seamless integration,and ensuring human safety with environmental sustainability.These advancements will drive further optimization and innovation in medical intelligent management systems.
