Clinical Efficacy of Modified Buyang Huanwu Decoction with Diclofenac Sodium Dual-release Enteric-coated Capsules in Treating Senile Chronic Pain with Qi Deficiency and Blood Stasis Type,and Its Effect on Pain-causing and Inflammatory Factor

Abstract
Objective To explore the clinical efficacy of modified Buyang Huanwu Decoction with diclofenac so-dium dual-release enteric-coated capsules in treating senile chronic pain with qi deficiency and blood stasis type,as well as its effect on pain-causing and inflammatory factor levels in serum.Methods A total of 82 elderly patients with chron-ic pain treated in the department of geriatrics,Nantong Hospital of Traditional Chinese Medicine from July 2021 to June 2023 were selected and divided into a control group(41 cases)and an observation group(41 cases)using the random num-ber table method.The control group was given diclofenac sodium dual-release enteric-coated capsules,while the obser-vation group was additionally treated with modified Buyang Huanwu Decoction.The treatment lasted for 2 weeks in both groups.The following aspects were observed and compared:clinical efficiency,adverse reaction;traditional Chinese medicine(TCM)syndrome scores,pain levels[visual analog scale(VAS),pain rating index(PRI),present pain intensity(PPI)],pain-causing and inflammatory factors in serum[prostaglandin E2(PGE2),endothelin-1(ET-1)],and quality of life[short form 36 health survey(SF-36)]scores before and after treatment.Results After treatment,both groups showed reduced TCM syndrome scores(fixed pain,limb numbness,scaly skin,shortness of breath,and fatigue)(P＜0.05),with lower scores in the observation group(P＜0.05).VAS,PRI,and PPI scores were decreased in both groups(P＜0.05),with the observation group exhibiting lower scores(P＜0.05).Serum ET-1 and PGE2 levels were reduced in both groups(P＜0.05),with lower levels in the observation group(P＜0.05).The role-physical,physical functioning,vitality,bodily pain,role-emotional,social functioning,mental health,and general health scores were improved in both groups(P＜0.05),with higher scores in the observation group(P＜0.05).The total clinical efficacy in the observation group(92.68％,38/41)was significantly higher than that in the control group(75.61％,31/41)(P＜0.05).Besides,no signif-icant adverse reactions occurred in either group during the treatment.Conclusion Modified Buyang Huanwu Decoction with diclofenac sodium dual-release enteric-coated capsules is effective in treating elderly patients with chronic pain of qi deficiency and blood stasis type.It can significantly alleviate symptoms and reduce pain,improving the quality of life in patients.Its mechanism may involve downregulating PGE2 and ET-1 levels.关键词
