参松养心胶囊治疗心衰合并室性早搏患者的疗效及作用机制
Efficacy and Mechanism of Shensong Yangxin Capsules in Treating Heart Failure Combined with Premature Ventricular Contraction

Abstract
Objective To observe the efficacy of the Shensong Yangxin Capsules on heart failure(HF)combined with premature ventricular contraction and to explore parts of its mechanism.Methods A total of 162 cases of HF patients combined with premature ventricular contraction admitted to the Northern Jiangsu People's Hospital between January 2022 and December 2023 were selected as the research subjects.Patients were randomly divided into a control group and an ob-servation group in a 1∶1 ratio by the random number table method,with 81 cases in each group.The control group received standardized Western medicine treatment,while the observation group was additionally given Shensong Yangxin Capsules.After 8 weeks,the following aspects were compared before and after treatment in both groups:the traditional Chinese medi-cine(TCM)syndrome score,Minnesota heart failure quality of life questionnaire(MLHFQ)score,6-minute walk test(6MWT),cardiac function[left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),left ventricular mass index(LVMI)],prematurity index(PI),the number of premature ventricular contractions,and blood indicators[blood N-terminal brain natriuretic peptide(NT pro-BNP),thromboxane B2(TXB2),endothelin-1(ET-1),nitric oxide(NO),matrix metalloproteinase-9(MMP-9),interleukin-6(IL-6),and tumor necrosis factor α(TNF-α)].Therapeutic effect and adverse reactions were also evaluated.Results After 8 weeks of treatment,TCM syndrome and MLHFQ scores were significantly decreased in both groups of pa-tients compared to before treatment,while 6MWT was significantly increased(P＜0.05),with the above changes being more pronounced in the observation group(P＜0.05).LVEF was increased in both groups,while LVESD,LVEDD,LVMI,and blood NT Pro-BNP were decreased(P＜0.05),with the above phenomenon being more pronounced in the observa-tion group(P＜0.05).PI was significantly increased in both groups,and the total number of premature ventricular contrac-tions was significantly reduced(P＜0.05),with the above changes being more pronounced in the observation group(P＜0.05).The NO level was increased in both groups,while TXB2 and ET-1 were decreased(P＜0.05),with the above changes being more pronounced in the observation group(P＜0.05).The serum MMP-9,IL-6,and TNF-α levels were decreased in both groups(P＜0.05),with these levels being lower in the observation group(P＜0.05).The total effective rate of TCM syndrome in the observation group was 91.36％(74/81),which was significantly higher than 74.07％(60/81)of the control group(P＜0.05).There was no statistically significant difference in adverse reactions between the two groups(P＞0.05).Conclusion Shensong Yangxin Capsules have significant efficacy in treating heart failure combined with premature ventricular contractions.The drug can alleviate symptoms,protect cardiac function,and inhibit ventricular remodeling,whose mechanism may be related to anti-inflammatory and endothelial function protection.关键词
