胃癌病人术后核心症状的识别及影响因素分析
Identification of postoperative core symptoms in gastric cancer patients and analysis of influencing factors

Abstract
Objective:To investigate the incidence and severity of postoperative symptoms in gastric cancer patients.To explore the network characteristics and core symptoms of postoperative symptoms in gastric cancer patients using network analysis methods,and to analyze the influencing factors of core symptoms.Methods:A cross-sectional survey was adopted.A total of 269 postoperative gastric cancer patients who were hospitalized in the department of gastroenterology at a tertiary grade A cancer specialist hospital in Hangzhou city from January 2024 to April 2024 were selected as the research subjects conveniently.The Chinese version of MD Anderson Symptom Inventory Gastrointestinal Cancer Module was used for the survey.The R software was used to conduct a network analysis of the postoperative symptoms in gastric cancer patients and determine the core symptoms.Logistic regression was used to analyze the influencing factors of the core symptoms.Results:The symptoms with the highest incidence among gastric cancer patients after surgery were restlessness during sleep(85.9%),fatigue(85.1%),and pain(83.3%).The symptom network had good accuracy and stability.The predictable values of the nodes ranged from 13.1%to 60.0%Distress showed the greatest predictability.Vomiting(rs=2.424),fatigue(rs=1.159),and distress(rs=0.780)were the three nodes with the largest strength centrality and were the core symptoms.Age,gender,and the surgical method were the main influencing factors for the occurrence of vomiting.The scores of the Nutritional Risk Screening 2002(NRS 2002),hemoglobin count,and pre-albumin count were the influencing factors for the occurrence of fatigue.Education background and the occurrence of postoperative complications was the influencing factor for the occurrence of distress.Conclusions:This study used the network analysis method to explore the postoperative symptom network of gastric cancer patients,and found that vomiting,fatigue,and distress were the core symptoms of gastric cancer patients after surgery.It is suggested that medical staff should identify the core symptoms as early as possible,sever the relationships between symptoms,and take predictive interventions for the main influencing factors of the core symptoms to improve the accuracy and efficiency of symptom management.关键词
