Effect of GnRH agonist addition for luteal support on pregnancy outcomes of fresh embryo transplantation with GnRH antagonist regimen

Abstract
Objective:To compare the effects of adding GnRH agonist(GnRH-a)to enhance luteal support with conventional drugs on pregnancy outcomes in patients with fresh cycle transplantation with antagonist regimen. Methods:A retrospective analysis was conducted on the clinical data of patients who underwent IVF/ICSI-ET with the antagonist protocol at the Department of Reproductive Medicine of JiNing No.1 People's Hospital from January 2023 to June 2024.The patients were divided into two groups as conventional group(n=165)and enhanced group(n=98)based on the different types of luteal support drugs used.Patients in the control group were given conventional luteal support drugs,while those in the enhanced group received extra 0.1 mg of GnRH-a on the 4th,6th,and 8th days after oocyte retrieval besides the conventional treatment regimen.General clinical data,ovulation induction regimen,laboratory indicators,and pregnancy outcomes were compared between the two groups. Results:There were no significant differences in age,infertility types and causes,body mass index(BMI),basal endocrine status,anti-Müllerian hormone(AMH)level,antral follicle count(AFC),gonadotropin(Gn)usage,hormone levels on the trigger day,the number of retrieved oocytes,M Ⅱ oocytes,the number of two pronuclei(2PN),the number of high-quality embryos,the number of transplanted embryos and the type of transplanted embryos between the two groups(P＞0.05).The embryo implantation rate,HCG positive rate and clinical pregnancy rate in the enhanced group were significantly higher than those in the conventional group(P＜0.05).There were no significant differences in the rates of OHSS,ectopic pregnancy,twin pregnancy and early miscarriage between the two groups(P＞0.05). Conclusions:GnRH-a addition to enhance luteal support is beneficial to the improvement of embryo implantation and clinical pregnancy rates in patients undergoing fresh embryo transfer with the antagonist regimen.关键词
