基于子宫内膜癌FIGO 2023分期下的淋巴结转移与分子分型相关性研究
Association study between lymph node metastasis types and molecular typing in endome-trial cancer under FIGO 2023 Staging

Abstract
Objective:Based on the FIGO 2023 staging system for endometrial cancer,to explore the relationship between lymph node metastasis types and molecular subtypes and the influence of clinicopathological features,so as to provide a basis for accurate diagnosis and treatment.Methods:A retrospective analysis was conducted on 326 EC patients who were path-ologically diagnosed after surgery in Gansu Maternal and Child Health Hospital from December 2022 to December 2023.The clinicopathological data such as age,body mass index,pathological type were collected.Molecular typing was detected by Sanger sequencing and other methods,and the lymph node metastasis type was evaluated and statistically analyzed.Results:Among the 326 patients,30 had lymph node metastasis.There was no metastasis in the POLEmut type.There were 14 cases(46.7%)of the NSMP type,6 cases(20%)of the MSI-H type,and 10 cases(33.3%)of the p53abn type.There were statistically significant differences in age,BMI,NSMP,p53abn,pathological type,cervical stromal infiltration,Lymphovascular space invasion,and ER expression between micrometastasis and macrometastasis(P<0.05).There were also statistically significant differences in age,pathological type,lymph node metastasis(LNM)loca-tion,and LNM type among patients with different molecular subtypes(P<0.05).Univariate a-nalysis showed that tumor diameter,p53abn,and cervical stromal infiltration were risk factors for lymph node macrometastasis,and no independent risk factors were found in multivariate analy-sis.Conclusion:Distinct molecular subtypes exhibit disparities in lymph node metastasis pat-terns.Concurrently,the types of lymph node metastasis are significantly associated with various clinicopathological factors.Among them,tumor diameter,p53abn subtype,and cervical stromal invasion are influencing factors for lymph node macrometastasis,yet they are not independent risk factors.关键词
