妊娠期糖尿病患者不良妊娠结局的影响因素研究
Study on influencing factors of adverse pregnancy outcome in patients with gestational diabetes

Abstract
Objective:To explore the current situation of adverse pregnancy outcomes in patients with gestational diabetes(GDM)and analyze its influencing factors.Methods:A descriptive analysis was conducted on the adverse pregnancy outcomes of 916 GDM patients who gave birth at a tertiary comprehensive hospital in Chongqing from January 2021 to Decem-ber 2023.Univariate and multivariate logistic regression were used to analyze the influencing factors.Results:The incidence of adverse pregnancy outcomes in patients with GDM was 24.0%(220/916),and multifactorial logistic regression analysis showed that age>35 years(OR=10.103),pre-pregnancy body mass index(24～28kg/m2:OR=2.493;>28kg/m2:OR=6.773),pregnancy weight gain>14kg(OR=2.773),≥2 pregnancies(OR=3.386),fasting glucose(OR=1.979),comorbid hypothyroidism(OR=2.001),insulin therapy(OR=12.166),mode of delivery(OR=5.559),blood uric acid(OR=1.005),serum albumin(OR=0.831),fibrinogen(OR=2.201)were independent influences on the occurrence of adverse pregnancy outcomes in GDM patients.Conclusion:Patients with GDM have a higher incidence of adverse pregnancy outcomes,with age>35 years,pre-pregnancy BMI>28kg/m2 and insulin therapy being the main risk factors,and maternal blood uric acid,elevated fibrinogen and low-ered serum albumin being significantly associated with adverse outcomes.Therefore,pregnancy management should be strengthened to optimize glycemic control and regular monitoring of rele-vant biochemical indicators in order to develop individualized interventions to improve maternal and infant outcomes.关键词
