基于fNIRS探讨调脑理肺针刺联合肺康复治疗脑卒中后肺功能障碍的机制
Mechanism of acupuncture for regulating brain and lung combined with pulmonary rehabilitation in the treatment of post-stroke pulmonary dysfunction based on fNIRS

Abstract
Objective It is to observe the efficacy of acupuncture for regulating brain and lung combined with pulmona-ry rehabilitation in the treatment of post-stroke pulmonary dysfunction,and to explore its potential mechanisms by functional near-infrared spectroscopy(fNIRS).Methods Sixty patients with post-stroke pulmonary dysfunction treated in Panyu Cen-tral Hospital Affiliated to Guangzhou Medical University from August 2023 to September 2024 were randomly divided into control group and treatment group,with 30 cases in each group.The control group was treated with pulmonary rehabilitation therapy,while the treatment group was treated with acupuncture for regulating brain and lung combined with pulmonary re-habilitation,both once daily,10 times as one treatment course,totally treated for 2 course,3-day interval between treat-ment sessions.The diaphragm end expiratory thickness(DTe),diaphragm end inspiratory thickness(DTi),diaphragmatic excursion quiet breathing(DEQB),diaphragmatic excursion deep breathing(DEDB),forced expiratory volume in one sec-ond(FEV1),peak expiratory flow(PEF),forced vital capacity(FVC),activation of left and right prefrontal cortex(LPFC,RPFC),and left and right premotor cortex(LPM,RPM)of patients in the two groups were observed and com-pared before and after treatment.Results After treatment,the DTe,DTi,QEQB,DEDB,FVC,FEV1,PEF were signifi-cantly were significantly higher than those before treatment in the two groups(all P<0.05),and the above indicators in the treatment group were significantly higher than those in the control group(all P<0.05);After treatment,the activation of LPFC,RPFC,LPM and RPM were significantly higher than those before treatment in the two groups(all P<0.05),and the activations in all regions in the treatment group were significantly higher than those in the control group(all P<0.05).Conclusion Acupuncture for regulating brain and lung combined with pulmonary rehabilitation can effectively im-prove pulmonary dysfunction after stroke,this may be related to strengthen the regulation of respiratory muscles after in-creasing activation of cortical areas such as prefrontal cortex and premotor cortex.关键词
