超声血流参数对胎儿脐带绕颈及宫内窘迫的预测价值
The Predictive Value of Ultrasound Blood Flow Parameters for Fetal Umbilical Cord Around the Neck and Intrauterine Distress

Abstract
Objective To analyze the predictive value of ultrasound blood flow parameters for fetal umbilical cord around the neck and intrauterine distress.Methods 100 pregnant women who had fetal umbilical cord around the neck and intrauterine distress detected by ultrasound examination admitted to the First People's Hospital of Zhengzhou from December 2022 to December 2024 were selected as the study group;Another 100 pregnant women who underwent prenatal examination and had healthy pregnancy status during the same period were selected as the control group.The study group was divided into the mild group(n=48),moderate group(n=33),and severe group(n=19)according to the severity of fetal intrauterine distress,and divided into the<0.6 cm group(n=21),0.60~0.79 cm group(n=54),and>0.8 cm group(n=25)according to the indentation depth of the fetal umbilical cord around the neck.All study subjects underwent ultrasound examination,and the ultrasound quantitative blood flow parameters[systolic-to-diastolic velocity ratio(S/D),resistance index(RI),pulsatility index(PI)of fetal umbilical artery(UA)and middle cerebral artery(MCA)]were compared between the study group and the control group,between the pregnant women with different indentation depth of the fetal umbilical cord around the neck,and between pregnant women with different severity of fetal intrauterine distress.Pearson correlation analysis was used to explore the correlations of ultrasound quantitative blood flow parameters with different indentation depth of the fetal umbilical cord around the neck and the severity of fetal intrauterine distress.Results The S/D,RI,and PI of UA in the study group were higher than those in the control group,while the S/D,RI,and PI of MCA were lower than those in the control group,and the differences were statistically significant(P<0.05).Among pregnant women with different indentation depth of the fetal umbilical cord around the neck,the S/D,RI and PI of UA followed this trend:<0.6 cm group<0.60~0.79 cm group<>0.8 cm group,while the S/D,RI and PI of MCA showed the reverse trend:<0.6 cm group>0.60~0.79 cm group>>0.8 cm group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that S/D,RI and PI of UA were negatively correlated with Apgar score,and positively correlated with indentation depth of the umbilical cord around the neck(all P<0.05).Conversely,S/D,RI and PI of MCA were positively correlated with Apgar score,and negatively correlated with indentation depth of the umbilical cord around the neck(all P<0.05).Conclusions Ultrasound blood flow parameters have close correlations with fetal umbilical cord around the neck and intrauterine distress.Elevated UA blood flow parameters and decreased MCA blood flow parameters indicate the presence of umbilical cord around the neck and intrauterine distress in the fetus,and the higher the indentation depth of the umbilical cord around the neck,the more severe the degree of intrauterine distress.关键词
