Research progress on the application of family doctor contracted services in elderly patients with chronic diseases
摘要
Abstract
The growing population of elderly patients with chronic diseases has diverse healthcare needs.As a key component of primary healthcare,Family Doctor Contracting Services(FDCS)can effectively help patients access high-quality medical services while promoting self-management and health maintenance.This review examines the concept of FDCS and its clinical effectiveness in elderly patients with chronic diseases,with a focus on its implementation and outcomes in managing prevalent chronic diseases inclu-ding hypertension,diabetes,and coronary heart disease.Additionally,the strengths and limitations of current FDCS applications are analyzed,along with proposed strategies for improvement.These findings may serve as a valuable reference for enhancing FDCS in the management of elderly patients with chronic diseases and informing future research.关键词
