Improved Conjunctival Approach to Lower Eyelid Bag Correction:Clinical Application and Evaluation of Free Orbital Septum Fat Backfilling

Abstract
Objective To evaluate the efficacy and safety of the conjunctival approach combined with orbital septum fat-free backfilling in treating lower eyelid deformities.Methods Clinical data from patients with lacrimal groove or infraorbital depression treated at Chengdu Xuanyan Medical Cosmetic Hospital between April 2020 and April 2023 were analyzed.The conjunctival approach was combined with orbital septal fat-free backfilling.Pre-and post-operative assessments included Tear Trough Rating Scale(TTRS)and Barton aesthetic ratings based on a postoperative satisfaction questionnaire.Results A total of 124 patients with bilateral lower eyelid pouches presenting lacrimal groove or infraorbital depression(10 males,114 females;age range 19-66 years,mean age 37.81±10.45 years)were included.Lacrimal groove malformations were classified as type I in 21 patients,type Ⅱ in 53 patients,and type Ⅲ in 50 patients.Within 3 to 5 days post-operation,patients experienced varying degrees of conjunctival edema,lower eyelid swelling,skin ecchymosis(24 cases),and conjunctival hemorrhage(10 cases).All patients recovered within 1 to 2 weeks without complications such as hematoma,diplopia,fat liquefaction,fat nodules,or fat embolism.Follow-up ranged from 6 to 12 months(mean 8.92±1.99 months).Significant improvements were observed in TTRS scores(P＜0.05)and Barton aesthetic ratings for type I and type II patients(P＜0.05).Overall satisfaction rates were high,with 40.3%very satisfied,54.8%satisfied,and 4.8%dissatisfied,resulting in a satisfaction rate of 95.2%.Conclusion The conjunctival approach combined with orbital septum fat-free backfilling is a safe and effective surgical method for correcting lower eyelid pouches,lacrimal groove deformities,and infraorbital area depression.This technique enhances facial aesthetics and improves patient satisfaction.关键词
