肺移植治疗特发性胸膜肺实质弹力纤维增生症1例并文献复习
Lung transplantation for idiopathic pleuropulmonary fibroelastosis:a case report and literature review

Abstract
Objective To investigate the efficacy of lung transplantation in the treatment of idiopathic pleuroparenchymal fibroelastosis(IPPFE).Methods The process of lung transplantation treatment for an IP-PFE patient was reported.The patient was diagnosed through lung biopsy.His symptomatic treatment was ineffec-tive and the symptoms worsened.After evaluation,lung transplantation surgery was performed,and clinical out-comes were observed during the follow-up period.Using"lung transplantation"and"idiopathic pleuropulmonary parenchymal fibroelastosis"as search terms,literature was searched in China National Knowledge Infrastructure(CNKI),Wanfang Data and the PubMed database until January 2024.The effectiveness of lung transplantation treatments were summarized for the reported IPPFE patients.Results The patient successfully underwent a continuous double lung transplantation assisted by veno-venous extracorporeal membrane oxygenation(VV-ECMO),with good postoperative recovery and smooth discharge.The patient had been followed up for 30 months and the postoperative recovery was good.Through precise literature search,a total of 12 English literature related to the clinical treatment of IPPFE were screened,including 1 review,3 monographs,and 8 case reports.No relevant Chinese literature had been found.42 patients ultimately included in the study underwent lung transplantation surgery,with an average overall survival of(80±35)months after surgery.Conclusions Lung transplantation is an effective treatment for end-stage IPPFE,which can improve the cardiopulmonary function,improve the quality of life and prolong the survival time.It is necessary to strictly control infec-tion and recurrence after operation.关键词
