微型游离皮瓣重建与超显微技术再植治疗Tamai Ⅰ区指端离断的疗效比较
Comparative efficacy of micro free flap reconstruction versus supermicrosurgical replantation for Tamai zone Ⅰ fingertip amputations

Abstract
Objective To investigate the clinical outcomes of micro free flap reconstruction versus supermicrosurgical replantation for fingertip amputations in Tamai zone Ⅰ.Methods A retrospective analysis was conducted on 47 patients with Tamai zone Ⅰ fingertip amputations who were treated between May 2022 and September 2023.According to the surgical procedures,the patients were divided into two groups:The replantation group(n=26)underwent supermicrosurgical replantation,while the reconstruction group(n=21)received micro free flap reconstruction.The clinical outcomes were compared between the two groups.Results All procedures were successfully completed with survival of replanted fingers/reconstructed flaps in both groups.No significant differences were observed in operative time or complication rates(P＞0.05).All patients were followed for 3 to 12 months.At 3 months postoperatively,functional assessment using the Disabilities of the Arm,Shoulder and Hand(DASH)questionnaire showed significantly better scores in the reconstruction group(13.0±5.6)compared to the replantation group(17.2±7.2)(P＜0.05).According to the Chinese Society for Surgery of the Hand(CSSH)functional criteria,outcomes were excellent in 8 cases,good in 15 cases,and poor in 3 cases for the replantation group,versus excellent in 15 cases,good in 5 cases,and poor in 1 case for the reconstruction group,with no significant difference in excellent-to-good rates(P＞0.05).Conclusion For Tamai zone Ⅰ fingertip amputations,micro free flap reconstruction may offer some advantages in functional recovery compared to supermicrosurgical replantation.关键词
