Abstract
Objective To compare and analyze the therapeutic efficacy,changes of accommodation and diopter in myopic astigmatic amblyopia which be treated with distance and near visual training.Methods A cohort study selected 92 patients with myopic astigmatic amblyopia who visited our hospital outpatient department and randomly divided into two groups,A and B.Among them,55 patients in Group A were 87 eyes(6.67±1.11)years old and 37 patients in group B were 62 eyes(6.76±1.23)years old.Group A received distance visual stimulation training with red visual chart,and Group B received CAM near visual stimulation training.The treatment period of all cases was 12 months,and the changes of corrected visual acuity,accommodative response and refractive power of amblyopic eyes were compared and analyzed in two groups of patients A and B.Results There was no statistically significant difference in pre-treatment visual acuity,diopter and accommodation between group A and group B(P>0.01).Comparative analysis of therapeutic effects in group A and group B Mean improvement in corrected visual acuity:Group A:0.42±0.19,Group B:0.31±0.16(t=4.58,P<0.01)51 eyes cured(58.62%),30 eyes effective(34.48%),and 6 eyes ineffective(6.90%)in group A.And 22 eyes cured(35.48%),32 eyes effective(51.61%),and 8 eyes ineffective(12.90%)in group B(x2=46.86,P<0.01).Changes in refractive error before and after treatment:Astigmatism:Group A:-0.08±0.15 D;Group B:-0.24±0.23D(t=-4.93,P<0.01);Spherical equivalent:Group A:-0.31±0.25D;Group B:-0.84±0.42D(t=-11.58,P<0.01).The statistical test showed a significant difference between Group A and Group B after treatment(x2=63.25,P<0.01).Changes in accommodative response of amblyopic eyes post-treatment:Group A:Eyes with normal accommodation increased by 65.52%,Eyes with accommodative lead decreased by 11.49%,Eyes with accommodative lag decreased by 54.03%.Group B:Eyes with nor-mal accommodation decreased by 1.61%,Eyes with accommodative lead increased by 8.07%,Eyes with accommodative lag decreased by 6.46%.Conclusion Distance visual stimulation training for myopia-astigmatic amblyopia is beneficial for improving therapeutic efficacy,enhancing accommodative function and controling axial myopia progression.关键词
