儿童基本型间歇性外斜视两种术式术后1年疗效比较

Abstract
Objective To compare the efficacy of bilateral lateral rectus recession(BLR)versus unilateral lateral rectus recession combined with medial rectus resection(R-R)in children with basic-type intermittent exotropia(IXT),and to analyze the impact of refractive status and angle of deviation on prognosis.Methods 70 IXT children with deviation angles of 20△～60△,treated between March 2021 and March 2022,were stratified by deviation angle(≤40△:BLR group,n=35;＞40△:R-R group,n=35).Outcomes at 1-year postoperatively included orthophoria rate,change in deviation angle(near/distance),fusion function recovery,stereopsis recovery,and recurrence rate..Results 1)Orthophoria:BLR group:97.14％(34/35);R-R group:94.29％(33/35)(P＞0.05).2)Deviation angle reduc-tion:Near:BLR group:32.53△±7.23△to2.11 △±1.82△;R-R group:48.31 △±6.52△to3.53△±2.44△(P＜0.05).Distance:BLR group:35.81 △±6.91 △ to 1.93△±1.64△;R-R group:51.21△±7.13△to 3.81△±2.14△(P＜0.05).3)Visual function recovery:Fusion recovery:BLR group 85.71％(30/35)vs.R-R group 68.57％(24/35)(P＜0.05).Stereopsis recovery:BLR group 80.00％(28/35)vs.R-R group 62.86％(22/35)(P＜0.05).4)Recurrence rate:BLR group 2.86％(1/35),R-R group 17.14％(6/35)(P＜0.05).Conclusions For IXT with deviation≤40△,BLR demonstrates superior binocular visual function recovery and lower recurrence rates,while achieving comparable orthophoria to R-R.However,R-R remains necessary for deviations＞40△.Surgical procedure selection should be optimized based on the angle of deviation,refractive status,and surgical dosage calculations.关键词
