基于Beers标准(2023版)的某院老年患者潜在不适当用药分析
Analysis of Potential Inappropriate Medication in Elderly Patients in A Hospital Based on the Beers Criteria(2023 Edition)

Abstract
Objective To evaluate the incidence of potentially inappropriate medication(PIM)in elderly patients in a hospital based on the Beers Criteria(2023 edition),and to analyze its influencing factors.Methods The medical records of elderly patients(≥65 years old)who were hospitalized in the First People's Hospital of Wuhu from January to June 2023 were selected.The clinical data of the patients were collected through the hospital information system.The PIM was evaluated according to the Beers criteria(2023 edition).Binary Logistic regression analysis was used to explore the influencing factors of PIM in elderly patients with multiple medication(≥5 kinds).Results A total of 1 562 cases of patients were included,with an average age of(75.17±6.67)years and an average number of medications of(13.10±5.78)kinds.A total of 738 cases(47.25%)and 2 266 case times were screened out.There were 499 cases(31.95%)and 1 404 case times(61.96%)of PIM related to drugs,45 cases(2.88%)and 50 case times(2.21%)related to diseases or symptoms,496 cases(31.75%)and 667 case times(29.44%)of drugs should be used with caution,30 cases(1.92%)and 30 cases(1.32%)and 30 case times(1.32%)of drugs should avoid interaction,and 95 cases(6.08%)and 115 case times(5.08%)of drugs not used based on renal function.Binary Logistic regression analysis showed that age[OR=1.023,95%CI(1.01,1.04),P=0.027],number of medications[OR=0.855,95%CI(0.83,0.88),P<0.001],creatinine clearance rate[OR=1.011,95%CI(1.01,1.02),P<0.001],type 2 diabetes[OR=0.246,95%CI(0.18,0.34),P<0.001]and atrial fibrillation[OR=0.303,95%CI(0.20,0.46),P<0.001]were all independent risk factors for PIM in elderly patients.Conclusion The incidence of PIM in elderly patients during hospitalization is high.The main reasons are the use of potentially inappropriate drugs and the use of drugs with caution.Clinical pharmacists can optimize the drug treatment plan based on the Beers Criteria(2023 edition),and use it as a basis to prompt physicians to streamline medication,thereby reducing the incidence of PIM in elderly patients.关键词
