Clinical Pharmacists' Practice in Promoting Rational Drug Use Under DIP Payment Mode

Abstract
Objective To analyze the effect of clinical pharmacists on rational drug use under the diagnosis-intervention packet(DIP)payment model.Methods Four kinds of diseases,including transurethral resection of the prostate(TURP)for prostatic hyperplasia,laparoscopic cholecystectomy(LC)for gallstones,heart failure and upper gastrointestinal bleeding were selected as experimental diseases.Clinical pharmacists intervened by analyzing irrational drug use,participating in the formulation of clinical pathways,conducting pharmaceutical ward rounds,and performing prescription reviews.The rational drug use evaluation indicators of patients(with 50 cases in each disease)were compared between before intervention(January to October 2022)and after intervention(January to June 2023)in Jialing People's Hospital of Nanchong.Results Before intervention,all four disease categories exhibited issues such as inappropriate antimicrobial selection and prolonged medication duration.Compared with before intervention,the average total amount of hospitalization,the consumption sum of drugs and the proportion of consumption sum of drugs in patients with gallstones,prostatic hyperplasia,heart failure and upper gastrointestinal bleeding were significantly reduced after intervention(P<0.05).After intervention,the use intensity of antibiotics in patients with prostatic hyperplasia decreased from 33.48 DDDs to 21.37 DDDs,and cholecystectomy decreased from 38.72 DDDs to 24.77 DDDs(P<0.05).Additionally,the rates of irrational drug use including inappropriate antimicrobial selection and prolonged medication duration was significantly reduced(P<0.05).Conclusion Under the mode of DIP payment,the participation of clinical pharmacists in the control of clinical rational drug use can reduce the total cost of hospitalization and the proportion of drugs,and improve the level of rational drug use in hospitals.关键词
