巴氏腺囊肿造口术后念珠菌感染患者抗真菌治疗预后分析
Prognostic analysis of antifungal therapy in patients with candida infection after Bartholin gland cyst mar-supialization

Abstract
Objective To explore the outcomes of antifungal therapy and related influencing factors in patients with Candi-da infection after Bartholin gland cyst marsupialization.Methods The clinical data of 124 patients developed Candida infec-tion after Bartholin gland cyst marsupialization in the Department of Obstetrics and Gynecology of Wuhan No.1 Hospital from September 2020 to September 2024 were retrospectively collected.The efficiency of antifungal therapy among patients with different general clinical characteristics,surgery-related factors,infection characteristics,and antifungal therapy were compared.Logistic regression analysis was used to identify the influencing factors associated with the ineffectiveness of anti-fungal therapy in patients with Candida infection after Bartholin gland cyst marsupialization,and the predictive efficacy was analyzed through the receiver operating characteristic curve(ROC)analysis.Results Among the enrolled patients with Candida infection after Bartholin gland cyst marsupialization,96 patients had effective treatment.Univariate analysis and multivariate Logistic regression analysis showed that advanced age(OR=2.045,95％CI=1.927-4.512),comor-bid underlying diseases(OR=3.183,95％CI=1.924-10.963),non-Candidaalbicans(OR=6.260,95％CI=1.597-24.542),fluconazole resistance(OR=4.674,95％CI=2.081-7.561),itraconazole resistance(OR=4.446,95％CI=2.321-9.599),formation of Candida biofilm(OR=4.558,95％CI=1.243-16.714),and failure to reach the recommen-ded drug dose(OR=2.002,95％CI=1.555-7.219)were all independent risk factors for the ineffectiveness of antifungal therapy in patients with Candida infection after Bartholin gland cyst marsupialization(P＜0.05).Combination drug use was an independent protective factor for the effectiveness of antifungal therapy in patients with Candida infection after Bartholin gland cyst marsupialization(OR=0.386,95％CI=0.124-0.500,P＜0.05).ROC curve analysis showed that the area under the curve for the combined prediction of the ineffectiveness of antifungal therapy in patients with Candida infection af-ter Bartholin gland cyst marsupialization by the above seven influencing factors was 0.867.Conclusion Age,the presence of comorbid underlying diseases,the characteristics of Candida infection,and the situation of drug application affect the treat-mentof patients with Candida infection after Bartholin gland cyst marsupialization.关键词
