基于多模态MRI探究血管性认知障碍的脑结构与功能影像学特征
Exploration of brain structure and function characteristics of vascular cognitive impairment patients based on multimodal MRI

Abstract
Objective:Using multimodal methods including voxel-based morphometry(VBM)and fMRI,to explore the imaging characteristics vascular cognitive impairment(VCI).Methods:A total of 110 VCI patients were assessed.Using the Montreal cognitive assessment(MoCA)scores,56 cases with cognitive impairment were included in the observation group and 54 without cognitive impairment were included in the control group.The differences of total intracranial volume(TIV),gray matter volume(GMV),and fractional amplitude of low-frequency fluctuation(fALFF)values between the two groups were compared.Finally,the above indices of different brain regions in the observation group were extracted for correlation analysis with MoCA score.Results:There were no statistically significant differences in age,gender or years of education between the two groups(all P>0.05).The TIV was(1 339.87±135.24)cm3 in the observation group and(1 341.57±134.02)cm3 in the control group,with no statistically significant difference between the two groups(P>0.05).The GMV was(518.62±48.85)cm3 in the observation group and(536.84±62.53)cm3 in the control group,with a statistically significant difference between the two groups(P<0.05).Compared with the control group,the observation group showed decreased GMV in the bilateral medial superior frontal gyrus,bilateral superior frontal gyrus and right anterior cingulate cortex,and increased fALFF values in the bilateral calcarine,bilateral superior occipital gyrus,left middle occipital gyrus and left cuneus.Conclusion:Multimodal MRI preliminarily reveals the distinct structural and functional neuroimaging characteristics of VCI patients,including decreased GMV in the frontal lobes and aberrant spontaneous activity in the occipital lobe,may contribute to further exploration of neuroimaging biomarkers and quantifiable diagnostic indicators for VCI in the future.关键词
