Abstract
Objective:To evaluate the DTI parameter for high-grade glioma(HGG)with IDH mutation.Methods:Image and clinical data of 177 patients with HGG were screened from glioma data set in public image database.22 cases with IDH mutation were included in the mutated group,and 155 cases without IDH mutation were included in the wild group.The diagnostic performance of fraction anisotropy(FA),mean diffusivity(MD),axial diffusivity(AD)and radial diffusivity(RD)in tumor enhancement,proximal edema and distal edema for the diagnosis of IDH mutation were analyzed.Combined test was performed using the parameters with good diagnostic performance to confirm the best method.Results:ROC curve showed that the five parameters with the highest AUC and Yoden index were FA value of proximal edema,volume ratio of enhancement to edema,RD value of proximal edema,RD value of distal edema and MD value of distal edema,and the sensitivities of the five parameters were 0.750,0.909,0.800,0.900,0.850,the specificities were 0.779,0.571,0.634,0.572,0.579,respectively.The AUC,sensitivity,specificity and Yoden index of the combined five parameters was 0.866,0.800,0.826 and 0.626,respectively.RD value of distal edema and the volume ratio of enhancement to edema showed the highest diagnostic performance by serial test,with the sensitivity,specificity,Yoden index of 0.818,0.816,0.634,respectively.Conclusions:DTI parameters have good diagnostic performance to distinguish IDH mutation of HGG,and a serial test could improve the accuracy a step further.关键词
