基于静息态fMRI探讨针刺干预慢性颈痛的中枢机制
Research on central mechanism of acupuncture in chronic neck pain based on resting-state fMRI

Abstract
Objective:To explore the central mechanisms of acupuncture in chronic neck pain(CNP)using resting-state fMRI.Methods:A total of 31 patients with CNP were enrolled,including 16 patients in the acupuncture group and 15 patients in the control group.The acupuncture group received treatment consisting of body acupuncture combined with scalp acupuncture,while the control group received simple treatment without acupuncture.Both groups underwent scoring using visual analog scale(VAS)and Northwick Park neck pain questionnaire(NPQ)and resting-state fMRI before and after treatment.The brain functional activities were analyzed using amplitude of low-frequency fluctuation(ALFF)and regional homogeneity(ReHo).Results:The acupuncture group had a significant decrease in VAS and NPQ scores after acupuncture treatment(both P<0.05).While in the control group,VAS and NPQ scores had no differences before and after treatment(both P>0.05).In the acupuncture group,the increase or decrease in neuronal activity in the prefrontal cortex,cingulate cortex,inferior temporal gyrus,cuneus,precuneus,parietal lobule,anteroposterior central gyrus and occipital cortex after acupuncture treatment were revealed(all P<0.05).While in the control group,the decrease in neuronal activity in the right insula,right lenticular putamen,right inferior frontal gyrus of the triangle,and right superior temporal gyrus were revealed after simple treatment(all P<0.05),and no brain regions with activated brain functions were found.Conclusions:Acupuncture intervention demonstrates efficacy in alleviating CNP.The cerebral cortex exhibits significant activity alterations pre-and post-acupuncture intervention,indicating that acupuncture modulates these brain regions associated with sensory-cognitive-emotional processes to mitigate pain and ameliorate negative emotions.关键词
