纵向应变参数评估乳腺癌蒽环类药物化疗相关早期心肌损伤及红黄煎剂干预效果的临床研究
A clinical study of longitudinal strain in evaluating early anthracycline-related cardiac toxicity in breast cancer patients and the efficacy of Honghuang Decoction

Abstract
Objective:Using left ventricle longitudinal strain(LS)parameters derived from speckle tracking imaging to early identify anthracycline-related myocardial toxicity in breast cancer patients.Additionally,this study aims to evaluate the clinical value of early intervention using Honghuang Decoction.Methods:A total of 38 females with breast cancer were enrolled.They were divided into a control group(16 cases)and an experimental group(22 cases).Both groups received conventional anthracycline chemotherapy for 4-8 cycles,and the experimental group also received Honghuang Decoction from the first day of each chemotherapy cycle until the seventh day after this chemotherapy.Transthoracic echocardiography was performed before chemotherapy,3 and 6 months after chemotherapy to obtain conventional examination parameters(LAD,LVEDd,mitral annular septum s',lateral wall s',SV,LVEF)as well as LS parameters(LSendo,LSmid,LSepi).All the parameters were compared between the two groups.Results:The absolute values of left ventricular LSendo,LSmid,LSepi exhibited a pattern of initial decrease followed by an increase before chemotherapy,3 and 6 months after chemotherapy,and these parameters essentially returned to pre-chemotherapy levels at 6 months after chemotherapy(all P>0.05).In the control group,LSendo,LSmid,LSepi showed a continuous decreasing trend,with significantly lower values at 6 months after chemotherapy compared to pre-chemotherapy(all P<0.05).LAD,LVEDd,septal s',lateral s',SV and LVEF of the two groups had no significant differences at 6 months after chemotherapy compared to before chemotherapy(all P>0.05).Conclusions:The left ventricle LS is more sensitive than traditional echocardiographic parameters in early detection of myocardial damage caused by anthracyclines.Oral administration of Honghuang Decoction during chemotherapy can improve early anthracycline-related myocardial toxicity.关键词
