Application value of dual-source dual-energy CT combined with radiomics in differential diagnosis of benign and malignant thyroid nodules

Abstract

Objective:To investigate the value of dual-source dual-energy CT combined with radiomics in differential diagnosis of benign and malignant thyroid nodules.Methods:A total of 118 patients with 128 thyroid nodules confirmed by pathology were retrospectively analyzed,inlcuding 60 benign nodules and 68 malignant nodules.All nodules were randomly divided into a training set(85 nodules)and a validation set(43 nodules).The clinical and imaging features and dual-energy quantitative parameters were collected.Clinical independent predictors were screened and the clinical model was constructed through multivariate logistic regression analysis.The radiomics features of non-enhanced and enhanced images were extracted,and the LASSO algorithm was used to select the optimal features to construct the radiomics model.The clinical independent predictors and radiomics features were selected to construct the clinical-radiomics model and a nomogram was drawn.ROC curve was used to evaluated the diagnosis efficiency,DeLong test was used to compare the AUC values of different models,and calibration curve and decision curve were uesd to evaluate the calibration of the model and its clinical benefits.Results:Nodule calcification,aspect ratio and normalized iodine concentration(NIC)in the arterial phase were independent predictors for malignant thyroid nodules and were used to construct the clinical model.The AUC values of the clinical model in the training set and validation set were 0.860 and 0.848,those of the radiomics model were 0.937 and 0.918,and those of the clinical-radiomics model were 0.967 and 0.937,the highest.DeLong test showed that there was a significant difference between the clinical-radiomics model and the clinical model in the training set(P≤0.05).The calibration curve showed that the prediction results of the clinical-radiomics model were consistent with the actual results,and the decision curve analysis showed a high clinical benefit.Conclusion:The clinical-radiomics model in the preoperative identification of benign and malignant thyroid nodules have high clinical value.
