慢性阻塞性肺疾病急性加重期患者中医证素分布规律研究
Study on the Distribution Patterns of Traditional Chinese Medicine Syndrome Elements in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease

Abstract
Objective:To investigate the characteristics of TCM syndromes and syndrome elements during the acute exacerbation phase of chronic obstructive pulmonary disease(AECOPD),and to provide a basis for optimiz-ing integrated traditional Chinese and Western medicine diagnosis and treatment.Methods:A retrospective analy-sis was conducted on the medical records of 200 AECOPD patients from the Third Affiliated Hospital of Beijing University of Chinese Medicine.Data were statistically analyzed using SPSS 27.0 software.Results:The TCM syn-drome characteristics of AECOPD patients during acute exacerbation were characterized by a mixture of deficiency and excess.Excess symptoms commonly manifest as coughing,expectoration of white phlegm,sticky sputum that is difficult to expel,rales in the lungs,dark red tongue,greasy coating,and taut and slippery pulse.Deficiency symp-toms often present as shortness of breath,dyspnea,low albumin levels,and deep and fine pulse.The top 5 affected organ locations were:lung,heart,kidney,spirit of the heart,and spleen.The top 5 pathogenic factors were:phlegm,fluid retention,qi deficiency,yin deficiency,and dampness.Young and middle-aged groups were more likely to exhibit the syndrome element of fluid retention compared with elderly and very elderly groups,while very elderly patients were more likely to show blood stasis syndrome element than elderly patients.Patients in the de-ceased group had a higher frequency of heart and blood deficiency syndrome elements compared with those in the survival group,with significant differences(P<0.05).Conclusion:During the acute exacerbation phase of AECO-PD,the distribution of syndrome elements shows a pattern centered around the lung,affecting multiple organs.Pathogenic factors such as phlegm,fluid retention,dampness,and heat coexist with deficiencies of qi,yin,and yang.Young and middle-aged patients are more prone to fluid retention upon admission,while very elderly pa-tients are more susceptible to blood stasis.Syndrome elements related to heart and blood deficiency can serve as important indicators for prognosis.关键词
