盐酸厄洛替尼片致胰腺癌患者非溃疡性舌背疼痛伴味觉异常1例不良反应分析
Erlotinib-Induced Non-Ulcerative Dorsal Tongue Pain and Dysgeusia in a Pancreatic Cancer Patient:A Case Report

Abstract
A 62-year-old male patient was admitted to a hospital with paroxysmal dull pain in the epigastric region between the xiphoid process and umbilicus,which had worsened episodically over 2 weeks without obvious cause.The clinical diagnosis was pancreatic malignant tumor.The treatment plan included percutaneous intratumoral radioactive seed implantation,followed by combination chemotherapy with gemcitabine+fluorouracil and gemcitabine+docetaxel,combined with recombinant human endostatin injection.No adverse drug reactions occurred during this period.Three months later,due to disease progression,the patient received a combined chemotherapy regimen of erlotinib hydrochloride+gemcitabine.On day 5 of medication,the patient developed dorsal tongue pain and discomfort,followed by inability to perceive sweet and salty tastes during meals.Clinical examination showed intact dorsal tongue mucosa without erythema,edema,or erosion,distinguishing it from common stomatitis or oral ulcers and indicating a neurological adverse drug reaction.The dosage of erlotinib hydrochloride complied with the drug label.After discontinuation,symptoms gradually resolved within 3 days with topical treatment,suggesting a temporal association with drug use.Re-administration led to recurrence of the adverse drug reaction within 3 days,consistent with the pharmacokinetics of oral erlotinib,implying a possible correlation with plasma drug concentration.The clinical pharmacist evaluated the causality as"definite"via the Naranjo adverse drug reaction Probability Scale(score=7,"probable").This case highlights the need to recognize erlotinib-induced dorsal tongue pain and dysgeusia for early intervention,providing a reference for clinical practice and rational drug use.关键词
