Effects of Tirofiban Combined with Neurointerventional Surgery on MicroRNA,Neuroinjury Markers and Cognitive Function in Patients with Acute Cerebral Infarction

Abstract
Objective:To explore the clinical efficacy of tirofiban combined with neurointerventional surgery in acute cerebral infarction(ACI)and its effects on microRNA(miRNA),neuroinjury markers and cognitive function.Methods:A total of 80 ACI patients treated a certain hospital from May 2021 to April 2024 were selected and divided into the control group and the observation group by the random number table method,with 40 patients in each group.Both groups received conventional therapy(intravenous thrombolysis,collateral circulation improvement,intracranial pressure reduction,neuroprotection,lipid regulation).Patients in the control group received neurointerventional surgery,while patients in the observation group were treated with tirofiban hydrochloride for injection on the basis of the treatment in the control group.The miRNA(miR-19b-3p,miR-25),neuroinjury markers[lipoprotein-associated phospholipase A2(Lp-PLA2),peroxisome proliferator activated receptor-γ(PPAR-γ),thrombospondin-1(TSP-1)],cognitive function[mini mental state examination(MMSE)score],clinical efficacy and complications were compared between the two groups.Results:Seven days post-treatment,miR-19b-3p,Lp-PLA2 and TSP-1 levels were decreased in both groups(P<0.05),with more significant reductions in the observation group(P<0.05).miR-25 and PPAR-γ levels were increased in both groups(P<0.05),with higher levels in the observation group(P<0.05).The scores of each dimension and the total score of MMSE were increased in both groups(P<0.05),with higher scores in the observation group(P<0.05).The total effective rate of clinical treatment in the observation group(95.00%)was higher than that in the control group(77.50%,P<0.05).There was no statistically significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion:Tirofiban combined with neurointerventional surgery enhances clinical efficacy,regulates serum miRNA and neuroinjury markers levels,improves cognitive function,and does not increase complication risk in ACI patients.关键词
