Intraoperative Real-Time Multi-Frequency Electrocochlear Imaging and Electrode Scanning in Cochlear Implantation

Abstract
Objective To report changes of cochlear microphonics(CM)during cochlear implantation,and the relationship between real-time ECochG monitoring and hearing preservation during cochlear implantation.Methods We recorded the duration of electrode insertion in 4 cochlear implant cases of different etiologies and ages.Changes in CM amplitude recorded from cochlear electrodes at 250,500,1000 and 2000 Hz were analyzed and compared to preoperative hearing thresholds,intraoperatively predicted hearing thresholds and postoperative hearing thresholds,and their relationship was analyzed.Results The duration of electrode insertion ranged from 99 to 265 seconds.CM amplitudes showed a pattern of continuous growth,with the maximum ranging from 14.4 μV to 115 μV.There was a positive correlation between the percentage of decrease in the CM amplitude during cochlear electrode placement and postoperative changes in hearing values.Conclusion Real-time intraoperative ECochG monitoring during cochlear implantation can provide valuable feedback to the operator and help predict postoperative preservation of residual hearing.However,more data are needed to establish the relationship between intraoperative ECochG and postoperative hearing changes.关键词
