An Analysis of Factors Affecting Outcomes of Endoscopic Myringoplasty in Children

Abstract
Objective The purpose of this study was to investigate the impact of various factors on anatomical and functional outcomes of myringoplasty in children.Methods Clinical data of 74 children(76 ears)who underwent myringoplasty at the Department of Otolaryngology Head and Neck Surgery,Capital Institute of Pediatrics Children's Hospital,from August 2019 to August 2023 were reviewed and analyzed.The patients were grouped according to gender,side of surgery,age,size and location of perforation,eustachian tube function,and the season of surgery.All children were followed up for more than three months postoperatively,and anatomical healing,functional ratings,and audiological outcomes were compared.Results The rate of surgical success varied among large,medium,and small perforations(P<0.05),but not affected by gender,side of surgery,age,location of perforation,eustachian tube function,or season of surgery(P>0.05).In the ROC curve model using age to predict anatomical outcomes,the AUC value was 0.724,with the Cut-off at 9.7,indicating a moderate diagnostic efficacy.The air conduction threshold improved to(24.36±6.11)dB HL,with the air-bone gap narrowed to(10.81±6.96)dB at three months,showing significant improvement compared to preoperative levels(P<0.01).Conclusion For children undergoing myringoplasty,age can impact on the rate of surgical success,although surgery should not be delayed solely based on age.The size of the perforation also affect outcomes.Cartilage is recommended as a graft material in pediatric myringoplasty.关键词
