脐血胆红素、免疫球蛋白G抗体效价和呼气末一氧化碳值对新生儿溶血性高胆红素血症的预测价值
Predictive Value of Cord Blood Bilirubin,Igg Antibody Titer and Etcoc in Neonatal Hemolytic Hyperbilirubinemia

Abstract
Objective To analyze the predictive value of cord blood bilirubin,IgG antibody titer and end-tidal carbon monoxide value(ETCOc)for neonatal hemolytic hyperbilirubinemia(NHB).Methods Fifty-five neonates with ABO hemolytic hyperbilirubinemia born in our hospital between July 2021 and October 2024 were enrolled as the NHB group.Fifty newborns with ABO hemolytic disease but without hyperbilirubinemia were selected as the control group(non-NHB group).The cord blood bilirubin,ETCOc,and IgG antibody titer levels in both groups of children were detected and compared,and the diagnostic efficacy was analyzed using the receiver operating characteristic(ROC)curve.Results Cord blood bilirubin levels and ETCOc in the NHB group were higher than those in the non-NHB group(t=8.714,18.960,P<0.05).The proportion of IgG antibody titers of 1:256 and≥1:512 was higher than that in the non-NHB group(P<0.05).The AUC of cord blood bilirubin,ETCOc,and IgG anti-A(B)antibody titers in predicting hemolytic NHB were 0.776,0.834,and 0.770 respectively.The AUC of the combined prediction of the three was 0.924,and the sensitivity and specificity were 81.82%and 92.15%respectively.Conclusion The proportion of cord blood bilirubin,ETCOc and IgG anti-A(B)antibody titer 1:256 and≥1:512 in children with hemolytic NHB is higher than that in ABO-HDN children without NHB.Cord blood bilirubin,ETCOc,IgG antibody titer all have good predic-tive value for the occurrence of hemolytic NHB,and their combination provides a better predictive effect.关键词
