两种输尿管软镜鞘对上尿路结石患者输尿管软镜清石效果及预后的影响比较
Comparison on the Effects of Two Flexible Ureteroscopic Sheathes on Stone Clearance Effect and Prognosis of Patients with Upper Urinary Tract Stones

Abstract
Objective To compare the curative effect of flexible vacuum ureteral access sheath(FV-UAS)and routine ureteral access sheath(UAS)combined with flexible ureteroscopic lithotripsy(FURL)and their influences on prognosis in upper urinary tract stones.Methods According to the random number table method,262 patients with upper urinary tract stones in the Urology Department of the hospital were divided into the observation group(135 patients,FV-UAS combined with FURL)and the control group(127 patients,UAS combined with FURL)between October 2023 and May 2025.The surgical indexes(lithotripsy time,decline value of hemoglobin,length of hospital stay),stone clearance effect[stone free rate(SFR)],inflammato-ry indexes[C-reactive protein(CRP),interleukin-10(IL-10),white blood cell count(WBC),tumor necrosis factor-α(TNF-α)],renal function indexes[blood urea nitrogen(BUN),serum creatinine(Scr),neutrophil gelatinase-associated lipocalin(NGAL),cystatin C(CysC)]and incidence of postoperative complications were compared between the two groups.Results The litho-tripsy time in the observation group was longer than that in the control group,the decline value of hemoglobin was greater than that in the control group,and the length of hospital stay was shorter than that in the control group(P<0.05).At 3d after surgery,the SFR in the observation group(85.19%)was higher than that in the control group(74.80%)(P<0.05),There was no sig-nificant difference in SFR between the two groups at 4 weeks post-surgery(93.33%vs 90.55%,P>0.05).while TNF-α,IL-10,CRP and WBC were lower than those in the control group(P<0.05).BUN,Scr,NGAL,and CysC were higher than those in the control group(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion FV-UAS combined with FURL can significantly increase stone clearance rate while reducing renal injury,attenuating postoperative inflammatory response,shortening hospital stay,and maintaining a favorable safety profile without increasing complication rates.关键词
